FORM INSTRUCTIONS

1. The first page of the form is to be printed and displayed with your model or exhibit. The second and third page of the form will be used by Lake Sawyer Hawks to ensure we have space for your exhibit and can contact you if there is a problem with your entry form. Entry forms with personal information will be shredded 30 days after the exhibit date or sooner. Email address will only be used to contact exhibitors regarding the 2018 Museum of Flight RC Flying Model Exhibit.
   
   Entry Forms will also be available at the Museum on the day of the exhibit. Please use the exhibitors entrance South of the Main entrance between 9:00 and 10:00 AM. After 10:00 AM use the Main Museum entrance. You may also print your forms on your printer and bring them to the exhibit with your model.
2. Completely fill out all text boxes and select appropriate buttons.
3. After completing the Entry Form, check for blank fields and create a PDF or XPS file and attach the file to an email and send to: stevemof@comcast.net. See instructions below.

Release and Indemnity Agreement

I state that I wish to participate in course and/or activities offered by the Lake Sawyer Hawks, Radio Control Model Flying Club, (hereinafter referred to as "Hawks").

I RECOGNIZE THAT ANY HAWKS CLUB ACTIVITIES MAY INVOLVE CERTAIN DANGERS. I certify that I am aware of all the inherent danger of Hawks Club activities including, but not limited to: weather conditions, inexperience, accidents or illness in remote areas without medical facilities, the actions of any other members, any participants or any other persons, all of which may result in personal injury, including death, property damage or other losses.

In consideration for the right to participate in LAKES SAWYER HAWKS CLUB activities, I HEREBY RELEASE THE HAWKS CLUB AND ITS DIRECTORS, OFFICERS, INSTRUCTORS, MEMBERS, AND THE MUSEUM OF FLIGHT FROM ANY AND ALL LIABILITY, CLAIMS AND CAUSES OF ACTION ARISING OUT OF OR IN ANY WAY CONNECTED WITH MY PARTICIPATION IN ANY HAWKS CLUB ACTIVITIES AND FURTHER AGREE TO INDEMNIFY AND HOLD HARMLESS THE HAWKS CLUB AND ITS DIRECTORS, OFFICERS, INSTRUCTORS, MEMBERS, AND THE MUSEUM OF FLIGHT FROM ALL LIABILITY, CLAIMS AND CAUSES OF ACTION WHICH I MAY HAVE ARISING FROM MY PARTICIPATION IN HAWKS CLUB ACTIVITIES. The terms of this agreement will serve as a release and indemnity agreement for my heirs, personal representative, and for all members of my family, including minors.

I further state that I am (19) years of age or older and legally competent to sign this release, that I understand these terms are contractual and not mere recital, and that I have signed this document as my own free act. Parents or legal guardians must sign for all persons under nineteen (19) years of age.

How to Create PDF or XPS File to Email to Lake Sawyer Hawks - Do this before you leave this form!

1. Check your Entry Form for blank fields. You will be using your device email app to send an email to Lake Sawyer Hawks after you attach your Entry Form PDF file.

In Windows 10, click on Print in your browser. In the printer menu, select "Microsoft Print to PDF" instead of your printer. Next select the location to save the PDF file and enter a file name in the File name box. Please use the following format for your file: yyyy_Lastname_Firstname.pdf, where yyyy is the current year. Example: 2018_Black_Steve.pdf

In Windows 7, click on Print in your browser. In the printer menu, select "Microsoft XPS Document Writer" instead of your printer. Next select the location to save the XPS file and enter a file name in the File name box. Please use the following format for your file: yyyy_Lastname_Firstname.xps, where yyyy is the current year. Example: 2018_Black_Steve.xps

In Mac OS X, click the File menu and select Print, or just hit command+P. Look for the "PDF" button in the lower left corner, click on that pull-down menu and select "Save as PDF". File name can use underscore, but not spaces. Please use the following format for your file: yyyy_Lastname_Firstname.pdf, where yyyy is the current year.

2. Click this link: Steve Ashmore, attach your Exhibit Entry Form, and send. If you only use WebMail on your device, you may have to open your email app and enter the email address.
